What is Neck Lift Surgery?

Neck lift surgery; is an operation performed to treat wrinkles and sagging on the neck and lower face. It can be done under general or local anesthesia. The basis of the operation is based on the stretching of the skin and subcutaneous muscle tissue. During the surgery, liposuction or procedures to remove sub-muscle fat tissue can be performed on this area.

Wrinkles and sagging on the face, which are seen in people with the advancement of age, also occur in the neck region. Face and neck problems can also occur at a young age. The reason for this is due to genetic factors and rapid weight gain and loss.

Neck lift surgery is applied to the patient due to reasons such as looseness of the skin in the neck area, sagging in the muscle structure due to gravity, jowl especially seen in women, and excess fat tissue in the neck area.

    Preparation Before Neck Lift Surgery

    The points to be considered before neck lift surgery can be listed as follows;

    • It is necessary to stop taking blood thinners.
    • If such drugs are taken before the operation, it is necessary to inform the doctor.
    • Not only blood thinners but also cold medicines should not be used.
    • Painkillers can be taken as needed.
    • It is necessary to stop the consumption of tobacco and alcohol products.
    • Eating and drinking should be stopped after midnight.
    • Even if the operation time is late in the day, it is necessary not to eat or drink anything after 06:00 in the morning.
    • If there are medications such as blood pressure and diabetes medication that must be used continuously, these medications can be taken with a small amount of water well before the operation. The doctor who will perform the surgery on time can be consulted

    • Neck lift surgeries are performed by following the steps below;
    • Neck lift surgery is performed under general anesthesia.
    • It is carried out on average between 2 hours and 3 hours.
      • In neck lift surgery, incisions starting from the front of the ear are opened, these incisions are extended from under the earlobe to the back of the ear. An incision is made from the hairy area to the bottom of the chin.
    • Loose skin and muscle tissue in muscle tissue in the neck area are removed. This area is made tenser.
    • The skin is made tauter and the neck is reshaped.
    • Pipes called “tiny drains” are inserted to prevent blood accumulation after the surgery.

      Postoperative Period

      The following situations occur after neck lift surgery;

      • After 4 hours to 6 hours after the operation, the patient can eat something and walk.
      • The drains placed during the surgery are usually removed after 1 day to 2 days.
      • The bandages made after the surgery stay for an estimated one week.
      • Mild pain that can be managed with painkillers after surgery is considered normal.
      • Swelling and bruises continue for 1 week, gradually decreasing and disappearing.
      • Post-operative scars are hidden in front and behind the ear, so it does not cause aesthetic problems.
      • After 6 months to 1 year, these scars become invisible at all.

      Is There a Scar Left Over From Neck Lift Surgery?

      A neck lift is an operation that is done to fix sagging in the area under the chin called the jowls. The incision starts from under the earlobe, turns behind the ear, and then enters the scalp behind the ear. The whole incision is hidden behind the ear and in the scalp. Stitches can be removed seven to ten days after the surgery. There are various techniques for this surgery. Platysma plication, limbrication, skin stretching, or deep plane technique are the most commonly used techniques. 

      In addition to these procedures, other operations such as fat removal with a second incision under the chin, liposuction, muscle modification to correct neck bands, and salivary gland removal may be required. When these are necessary, a second, three-to-four-centimeter-long incision is needed under the chin, but since the incision here is matched to the chin wrinkles, it usually becomes undetectable after the surgery.

      Neck Lift Recovery

      Neck lift surgery is an operation that takes about two to three hours. After the operation, swelling and bruises occur on the neck and under the chin, but after the third day, these begin to decrease rapidly. Following the operation, the patient is dressed and a drain is placed. The reason for placing a drain is so that blood does not accumulate in the surgical field and healing can be accelerated. On the second day, the drains are removed, and the patient is usually discharged on the second day of the surgery. If the face and eyes are also involved, it is better for patients to bring a pair of sunglasses when they are discharged. You can take a bath three to four days after the operation. Numbness that occurs after surgery and lasts for several months is usually normal. 

      On the seventh or tenth day, swelling and bruising are minimal. On the tenth day, the metal sutures behind the ear are removed the next day, and the patient can return to work. For the first two weeks, the patient is not recommended to do things that require constant movement, such as heavy exercise, sexual activity, and housework. It is better to stay away from saunas, steam, and alcohol for the first three months. It is recommended that you refrain from smoking for one week before and three weeks after the surgery. Most of the time, people who smoke are the ones who have problems with skin necrosis after surgery. It is recommended to the patients that they use sunscreen cream for six months after surgery.

      How long does the neck lift surgery take and what type of anesthesia is used?

      A neck lift is a type of jaw cosmetic surgery. The aim is to make the jawline clearer and the jaw-neck angle sharper. For this purpose, many procedures are performed on patients today. In addition to neck lift surgery, jowl liposuction, jawline filler applications, and jaw prostheses are performed, or these procedures can be combined with neck lift surgery. If the patient has decided for neck lift surgery, what will be done in this surgery and what technique will be used directly affect the result of the operation, the recovery process, the duration of the operation, and the type of anesthesia to be applied to the patient. A simple skin removal and platysma application procedure can be performed under local anesthesia, and the procedure usually takes an hour on both sides. 

      However, if a deep plane neck lift, fat removal with an incision under the chin or platysmoplasty (tightening the muscle tissue under the skin with an incision under the chin),  and salivary gland removal will be applied, these procedures are performed under general anesthesia, and the operation takes approximately three hours. In other words, the need of the patient in neck augmentation surgeries and the treatment that should be applied to the patient are the first factors in determining this type of anesthesia and the duration of the operation.

      Who Is Eligible for Neck Lift Surgery? Is there an age limit ?

      Neck lift surgery is done on people who have lost their jaw line, have sagging skin on their chin and neck, have neck bands, have a lot of fat on their neck, or have sagging skin because they’ve lost too much weight. The long-term effects of smoking and the sun on our skin cause the skin and structures under the skin in our necks to sag over time. It may show up in the neck area because the salivary gland twists too much and the structures that hold it in place start to come apart. Furthermore, as we age, the submuscular fat pad in our midline of the neck may expand, and bands and wrinkled lines on our neck may appear. We recommend neck lift surgery to treat such conditions. 

      There is no age limit. It can be done at any age, but this patient group is usually not young. Because at young ages the jawline is prominent and the jaw-neck angle is sharp, In the middle-aged group, the problems are still mild, and these problems can be eliminated by methods such as liposuction and lipolysis. However, there may be sagging of the skin in the middle-aged group due to losing excess weight in a short time, and in such cases, the need for neck lift surgery may arise. Because of this, the procedure is usually done on older people, but there is no upper age limit.

      When a neck lift is performed under local anesthesia, patients do not feel any pain due to the effect of local anesthetic. After the operation, intravenous painkillers are given during hospitalization,  and the patient spends the first day comfortably. On second day, the pain decreses, and the patient is discharged with oral painkillers while being discharged home .

      Methods Used in a Neck Lift

      When planning a neck lift for a patient, the patient is evaluated with his/her whole face. Generally, this surgery is combined with a facelift. When people have jaw retardation, their necks start to sag at a younger age, and a jaw prosthesis can be recommended for these people. There are different methods used in neck lift surgery. The simplest of these is skin stretching. It is made by separating the skin from the underlying muscle tissue and removing the excess part, it is not used much today. Another method is muscle plication. In this technique, the skin is separated first, and then the muscle tissue is tightened by suturing the muscle tissue in several places. It creates tension in the muscle. Then the excess skin tissue is removed and the wound closed. In terms of risk, it’s a safer operation, and facial nerves almost never come up. 

      In the deep plane facelift method, the muscle tissue under the skin is cut. This muscle tissue is then separated from the tissue below it and stretched with stitches. It is a bit more risky than plication because the facial nerves pass under this muscle tissue, but it is a more effective and long-acting method in terms of effect. When done with care, the possibility of complications is not very high. This method is generally preferred for elderly patients and patients with advanced sagging. 

      When a patient has too much fat under the platysmal muscle, a neck lift is done along with fat removal through an incision under the chin. In elderly patients with neck bands, this under-chin incision can be used to create a tight muscle tissue that creates a corset effect on the neck by suturing the two platisma muscle edges together under the skin.

      Is the Result of Neck Lift Surgery Permanent?

      Neck lift surgery is a permanent procedure. During the surgery, the sagging tissues are fixed to the hard area behind the ear. However, the surgical technique is important here. Procedures such as skin stretching and application, for example, do not provide long-term results in severe sagging problems. In a few years, sagging will occur again. The deep-plane neck stretching method is the method with the highest permanence today. However, no treatment method has yet been found to stop the aging process. Therefore, after these surgeries, age goes backwards, but aging continues from where it went. Our skin’s elasticity declines over time, the skin stretches, and as it stretches, new sagging develops in the skin and the tissues beneath it. It is impossible for us to stop this process

However, in procedures such as open neck tear removal and deep plane neck lift, a drain is usually placed in the neck, followed by hospitalization for 1 day, after which the drains are removed and the patient is discharged the next day.

      Which Operations Can Be Combined with a Neck Lift ?

      Neck lift surgery can be combined with all operations on the face. However, it is most often combined with a mid-face lift, chin prosthesis, and jowl liposuction surgery. Since the patient group that usually has this surgery is advanced in age, it can be combined with other operations performed on elderly patients, such as eyelid aesthetic surgery, a face lift, and eyebrow lifting surgeries.
