      How is Bichectomy

      Application Performed?

      Bichectomy is also known as “cheek thinning”. Bichectomy can be applied by following the steps below;

      • Local anesthesia is first applied to the patient.
      • Mouth cleaning should be done thoroughly. Oral care before bichectomy is very important to prevent possible infections.
      • Small incisions are made near the fat bags on the cheeks.
      • Fat tissue is taken from the opened incisions.
      • Opened incisions are closed by suturing.
      • Afterward, the application is completed with a compressive bandage.

      Bichectomy application takes approximately 30 minutes to 45 minutes.

      • The patient can be discharged on the same day.

      Bichectomy surgery Permanence

      Bichectomy in Turkey is the removal of a part of the fat pads, which we call the “bichat fat pads,” located in the vicinity of the blowing muscle on both cheeks. This surgery is also called “Hollywood cheek surgery.” This procedure can be performed under local anesthesia or general anesthesia. The procedure takes about 30 to 45 minutes and is a permanent procedure. 

      After the procedure, edema develops on the cheeks for a few days, which may last 1-2 weeks. The desired effect hasn’t happened yet, even after the edema has subsided. Between 3-6 months, the lower soft part of the cheek, which is immediately under the cheekbones, thins a little, making the cheekbones appear more prominent. 

      Facial thinning is not always possible with bichectomy surgery alone. There are many factors, such as the salivary glands, the masseter muscle, the patient’s skin thickness and subcutaneous adipose tissue, and maxillary bone mass. If only the fat pad on the cheeks is removed, which goes all the way to the temples, the face might not always look much thinner.

      Bichectomy Recovery?

      There is edema on the face for a week following bichectomy surgery. If this edema is unilateral and painful, it should not be thought of as swelling after every bichectomy operation, and you should definitely consult your doctor. I would like to specifically mention two complications that can occur after bichectomy surgery. 

      One is a hematoma (blood pooling), and the other is an abscess. Hematomas happen when blood vessels are damaged during surgery and cause bleeding in the surgical area. Because of the stitches, the blood builds up on the cheek instead of flowing out, making the cheek swell. 

      There may be minor mucosal bleeding after each bichectomy procedure, but this is not a problem. The solution is to open the stitches again, find the bleeding source, and control the bleeding. In my own practice, I tell my patients: “After bichectomy, I wrap the entire head for 1 day in order to minimize the possibility of bleeding, putting pressure on the cheeks.” Good nutrition after surgery is also important for this surgery. 

      We generally recommend that patients be fed transparently for 2 days after the surgery. In other words, we recommend that they be fed little but often with smooth, transparent foods close to water. These foods include compost, fruit juice, broth (not hot), and milk.

      This results in the formation of the second complication I mentioned, an abscess. Unilaterally significant asymmetric swelling, pain, and fever may bring abscess to mind. If such feared events do not occur, swelling begins to decrease after the second week. After 3-4 months, the face thins out in the lower part of the cheekbones.

      Fat removal in bichectomy surgery

      In bichectomy surgery in Turkey, the fat is removed from the cheek portion of the bichat fat pack. We do not remove the part that extends to the temples. This may vary among individuals. 

      In my own practice, I extract different amounts of fat from every individual, ranging from  2 cc to 3.5 cc from a single side. 

      The surgery can be done with small incisions made in the mouth under local anesthesia, or these fat deposits can be removed during facelift surgery. Rather than removing it, the surgeoun replaces it and augments it during facelift surgery.

      Reinjecting removed body fat

      Bichectomy is considered oral surgery. A 1-cm incision made above or below the place where the salivary gland opens into the mouth is usually sufficient for this. However, when the fats are removed from the mouth, they are contaminated with saliva. There are billions of beneficial bacteria in the mouth. However, if these bacteria pass from the oral cavity to the blood or body cavities, they become our enemies. 

      Our face is one of the most commonly injected areas of the body. We inject fat from other parts of the body in order to fill under our facial skin and subcutaneous tissue, which loses volume as we age, to gain volume and benefit from the rejuvenation effect. We need to pay close attention to sterility in this process. 

      In general, these areas can be the abdomen, hips, or arms, but we do not use the fats from bites contaminated by the mouth. Because no one wants to risk infection by injecting contaminated fats into a face that already has millions of bacteria on it.

      Bichectomy procedure details

      Bichectomy in Turkey takes about 30-40 minutes. A few cc of local anesthetic keep us from feeling any pain during the procedure. Therefore, I perform the application under local anesthesia in my own clinic. However, it can be done under general anesthesia when people with diseases such as panic attacks and asthma are present. It can be done in the hospital under general anesthesia if it is done along with other surgeries.

      • Is there any pain felt during the operation?

      Patients do not feel pain during the bichectomy surgery because local anesthetics are put on their cheeks before the procedure. The procedure is extremely comfortable in the patient’s sitting position. 

      Thanks to the intramuscular injection of painkillers after the procedure, the patient does not have much pain for 7-8 hours (this is not necessary, oral painkillers can also be given), and the patient can overcome the next process with oral painkillers. On the second day, there is not much pain without taking painkillers.

      • Bichectomy surgery dietary restrictions 

      After a bichectomy, we recommend that patients eat smooth, liquid foods like compost, fruit juice, broth, and milk for 2 days and rinse their nets with mouthwash after each meal. This keeps food from getting into the hole left by the fat removal and causing an infection. 

      In the next 2–3 days, soup and soft foods are started, and then normal nutrition can be started 5 days after surgery. Otherwise, complications such as infection and an abscess may develop. If an abscess forms, it should be drained right away and treated with antibiotics that work on a wide range of bacteria.

      • Is there an age limit for bichectomy surgery?

      There is no age limit for bichectomy in Turkey. However, a 10- or 15-year-old child who requests a bichectomy surgery never sees a doctor. No doctor would perform such a procedure, even if there were such a thing. This is because the patient does not yet have the capacity to make decisions regarding changes to his or her own body. 

      In our country, this ability can be acquired legally at the age of 18, but in my own experience, I believe this process is more suitable for people in the 23–24 age range and older, who have finished the stages of accepting their own bodies.

      The cost of a bicornectomy is lower in Turkey for a variety of reasons, but the main one is that the country’s government is working hard to establish Turkey as a top destination for medical tourism worldwide by providing high-quality care at reasonable costs. Bichectomy surgery in Turkey will cost between 40 and 65 percent less than in Europe or North America. 

      In general, the cost of living in Turkey is low. Travel, food, and housing are all affordable.

      As a result, the cost of medical services in Turkey is 4-5 times less than in the USA and 2-3 times less than in Germany, Austria, Spain, South Korea, or USA. The cost of a bichetomy is included.

      Is Bichectomy and Hollywood Cheek the same procedure?

      Bichectomy is also referred to by other names, including Hollywood cheeks, cheek fat removal, buccal fat removal, and cheek thinning procedure.

      Can bichectomy be combined with other surgeries?

      Bichectomy can be combined with a cheek filler procedure or a fat transfer procedure to give the cheeks a more contoured appearance. A jawline procedure can also help you have a more contoured and attractive jaw after cheek thinning surgery.

      What should the patient do after Bichectomy in Turkey?

      Following the bichectomy, patients should follow a liquid diet for the first few days before introducing soft foods and gradually transitioning to a normal diet. The wounds should heal within two weeks.

      Is it necessary to stay in the hospital after the procedure?

      Bichectomy in Turkey is a one-day procedure. No hospitalization is required. After the surgery, the patient can stay in the clinic for two to three hours before being sent home. If the procedure needs general anesthesia, the patient must stay still for at least 4 hours.
      After 4 hours, the patient is fed liquid food. If the patient’s vital signs and urine output are normal, he or she will be sent home.
